HOW TO ENROLL YOUR CHILD
Download the associated enrollment forms from this site or
stop by the center where you will be enrolling your child and ask for an
enrollment packet. Complete the forms, have the Medical Authorization
Form notarized, attach your enrollment fee as well as your first week's
tuition payment per child and drop the completed and notarized forms back
off at your child's center. Or mail your enrollment for and check
to: BOFSP, Inc., 801 W. Beacon Road, Lakeland, FL 33803.
Once we receive your child's completed and notarized
enrollment packet and all fees due, your child can start immediately.
