HEALTH
WATCH - Please note for the safety of all the children and staff, if
your child shows signs of flu-like symptoms, please keep your child
home until all symptoms including fever have been gone without
medication for at least 24 hours.
ACCIDENT
INSURANCE
Accident insurance coverage is
built into your registration fee, and if an emergency occurs we'll take
action to ensure the safety and health of your child.
Every attempt will be made to contact you, the
parent, guardian or designated person in charge in the event of an
emergency. However, in the event of a serious emergency the Polk County
Emergency Medical Service (911) will be called to administer treatment
and transport to the nearest emergency medical facility for treatment.
Please note that we cannot transport any child for any medical
treatment.
We will inform you about any
injury or accident. Our insurance policy is an "excess
coverage" policy which means that your primary insurance must pay
first. This insurance will pay for any charges not covered by the
primary insurance.
Accident insurance claims are available from
the Director at the center.
If you seek medical attention for your child,
it is your responsibility to return the completed insurance claim forms
to the Director at your child's center so they may be forwarded to the
proper insurance companies.
REPORTING INJURIES
All injuries, no matter how small, must be reported to the
adult in charge immediately. Please help us by teaching your child to
always report it to a teacher if they have any type of injury occur or
get hurt in any way.
ILLNESS
If your child becomes ill during the school day, we will
notify the parent or guardian will be asked to come pick up the
child.(Children with head lice cannot attend until their hair has been
chemically treated and completely cleared of all eggs, nits, and bugs.)

IS CALLED FOR ALL EMERGENCIES!
If
a child shows signs of illness with fever (101 F or above), vomiting,
diarrhea, constant cough, heavy nasal discharge, red eyes or irritability
do not send your child to the center. This is for the protection
of your child as well as the others. If any of these signs appear while
the child is in the center, the Director will call the parent to come
pick up the child. If you are called please come as quickly as possible
as illness may be contagious. When sent home from the center,
the child must be free from all illness for 24 hours before returning
to school. This includes diarrhea and fever. This is a policy set by
the Polk County Health Department and the Department of Children
and Families.
Every
child is to have a current physical and shot record before he/she can
start. These are to be kept updated as required by DCF. Physicals must
be renewed every two years.
Any
child who is not up to date on immunizations, physicals
or
medical requirements will have to be withdrawn from the center until
requirements are met.
All
children sent home with head lice must have their scalp
and hair
treated for lice, washed, and
cleared
before returning to the center. This
means all eggs,
nits, and bugs must be
combed out of the hair completely and
hair cleared of all bugs, nits, and eggs before being allowed to return.
MEDICATION
Medication
will not be administered at this center. Please arrange for your child's
doctor
to prescribe meds that are given perhaps every
8 to 10 hours and/or
parents will
need to come
to the center to give any medications
their child may need. Please inform the center when
your child is on medication.
ABSENCES
Please
notify the
center in
the event your child will be absent or other pick up arrangements are
made on the days when this change is in effect. This will help us account for all
children at roll
call.
